Issue Description
On the Agency Management Table, when clicking Request Update for any agency entry, the modal that appears does not align with the Lovable design specifications. The modal card size, font size, and overall UI styling are inconsistent, resulting in a UI mismatch.

Steps to Reproduce¶

1. Sign in as an admin user.
2. Navigate to the Agency Management Table page.
3. Click on Request Update for any agency entry.
4. Observe the modal that appears.
5. Compare the modal’s size, font size, and overall UI styling with the Lovable design.

Expected Result
The modal card size, font type, font size, spacing, and UI elements should be consistent with the Lovable design.

Actual Result
The modal styling does not match the Lovable design specifications.
